1. Full name and phone number of the recipient

2. If you want to PICK up a physical gift card that will be packaged for you. If you prefer a paperless gift card it will be added directly into the recipient's file on our Point of Sales System.

*gift cards are not redeemable for cash, and cannot be used to purchase another gift card. Any resale of transfer or this card will render it void and subject it to cancellation.
